Kaiser Permanente to use AM-PAC in major stroke study:
A computer assisted test (CAT) application of the Activity Measure for Post- Acute Care (AM-PAC CAT) is being integrated into the Kaiser Permanente Northern California (KP) information network to characterize the rehabilitation outcomes for stroke patients across the continuum of care in all post-acute care (PAC) sites. The project will utilize the Kaiser Permanente Functional Outcome System (KP-FOS) to analyze data across different post-acute settings, particularly IRF and SNF settings, to identify the most effective post-acute care approaches to promote functional recovery after stroke. When stroke patients leave the acute hospital they travel care paths through various PAC sites such as inpatient rehabilitation hospital (IRH), skilled nursing facility (SNF), or home health (HH). However their families and providers do not have a way to assess their care as they move from site to site. Investigators will use the KP-FOS to measure rehabilitation outcomes for 244 stroke patients, following them for six months post-stroke and measuring their rehabilitation outcomes at each care site. The assessment scores will be integrated with patient data from the KP electronic medical record and analyzed using the new reports.
